1. Select the decade from the drop-down menu (must be the same decade for all locations).
   
2. Choose the file that contains your recipient list (acceptable file formats include: Microsoft Excel, CSV, and Tab Delimited)
3. Make sure that the columns in your file match those in the format below. For a recipient list template in Microsoft Excel format, click here.
4. Enter the quantity of decade boxes being ordered and click "Add to Cart." (Quantity will equal the number of recipients on your list.)
   

Here is a list of the columns to upload: (red = required)

1. First & Last Name
2. Company
3. Address 1 (street address, no PO boxes)
   
4. Address 2 (apt, suite, floor, mail stop, etc.)
   
5. City
6. State (continental US only)
   
7. Zip Code
8. Phone (in case UPS has a question)
   
9. Gift Message (15 words or less)
